Spring Camp Registration Form
Welcome & thank you for your interest in joining the Colonia Soccer Spring Break Camp...

Please fill out the following form and then click "Submit" at the bottom of the page.  The camp will take place (Monday April 1st - Thursday, April 4th) at the Colonia High School Turf Field from (10:30AM - 1:00PM).

*Reminder that in order to be successfully registered for Camp you need to do the following:
-  Complete Registration Form online.
-  Submit $150 payment to Thomas Joraskie to the address listed below or via Venmo.
-  Confirmation email will be sent from Coach Joraskie once all the information above is received.

Liability Waiver
Liability Waiver:  I hereby acknowledge that I have voluntarily and freely elected to participate in the Colonia Soccer Camp, Colonia Soccer LLC.  Participation in the Colonia Soccer Camp has some inherent risks and injury can occur.  On rare occasions these injuries can be serious.  In consideration of my child being allowed to participate in the Colonia Soccer Camp/Colonia Soccer LLC I, the parent/guardian, assume the risk of all injury and agree not to sue the Colonia Soccer LLC, the camp directors, coaches, assistant coaches, or volunteers for any and all injuries caused by or resulting from participating in the Colonia Soccer Camp.  The camper is physically fit to participate in all camp activities.  By signing this waiver, I also authorize the use of pictures of the above-named participant to be posted on Social Media and websites for promotion and advertising.
