Step 1. Fill out this Google Form - CHIS Music Program.
Step 2. Complete the School Site Volunteer Form.
Step 3. Mandated Reporting Certificate -
a. All school volunteers, including new, current and returning volunteers, must complete the required Mandated Reporter Training annually before volunteering on campus or participating in school activities per California Senate Bill 848 (SB 848).
b. Visit https://mandatedreportertraining.com/california/ to complete training.
c. Once completed, the certificate must be submitted along with the volunteer application, TB and Livescan form.
Step 4. Fingerprinting - Livescan (if you have submitted your approved fingerprints livescan to CNUSD in previous years, you can by place this step.
a. Fingerprinting is available through any approved Livescan location, the cost is the responsibility of the applicant.
b. Livescan locations may be found online.
c. The District Office has no control over fingerprint processing times from FBI/DOJ and can take up to 45 days to receive at our office.
Plan Ahead a. Field trips, dances and other chaperoning events require time to plan. Applications are to be submitted early in the school year to ensure adequate processing time. Last minute requests may not be accommodated and may inhibit proper parental supervision for events.
