Instructions for Using This Assessment Template

To use: Download a copy of this worksheet (File > Download)

1. When using this spreadsheet, add data to white cells only and let the formulas in the yellow cells do the rest!

2. In the Procurement Assessment tab, list all goods and services procured to directly support your organization's internal operations as well as where they are procured from (supplier).

3. Self-assess known risks using lists and databases provided by reputable entities as relevant to your organization's context and industry (see examples in the Program Manual).

4. Indicate whether the supplier of each good or service contributes to the metric criteria in Columns F and G.

5. Enter the estimated annual spend of each good or service in Column H.

6. Enter your organization's estimated total annual spend on all goods and services listed in Cell L2.

7. If pursuing Level 3, complete Columns D, E, and F in the Due Diligence tab. See examples and resources in the Program Manual. (Columns A, B, C, and H will automatically copy from the Procurement Assessment tab.)

8. If pursuing Level 4, additionally complete Column G in the Due Diligence tab.

9. When you have completed your calculations, please upload this spreadsheet in Apply.
