Description Measuring the number and percentage of adult patients served by contracted health systems who are at high risk of cardiovascular events and who have managed their cholesterol with statin therapy in participating health care systems/clinics that provide primary care to adult patients 18 to 85 years old. IDPH contracts with willing employers, health systems and others that provide health care to Iowans to implement policy and systems changes that will improve cholesterol management. Formula The numerator for this measure is the number of adult patients served by contracted health systems who received at least one order for statin therapy during the measurement period. The denominator is the number of adult patients served by those same contracted health systems who meet criteria to be considered at high risk for cardiovascular events. Description Measuring the number and percentage of adults 18 to 85 at partcipating health care systems/clinics with known high blood pressure who have achieved clinical blood pressure control. IDPH contracts with willing employers, health systems and others that provide health care to Iowans to implement policy and systems changes that will improve high blood pressure control. Formula The numerator for this measure is the number of adult patients served by contracted health systems with a diagnosis of high blood pressure whose blood pressure has been adequately controlled.
