4 | Goal To increase the number of WISEWOMAN (WW) participants who are screened at clinics that have team-based care policies in place to improve cardiovascular disease outcomes. Team-based care is a health care system strategy to enhance patient care by having two or more health care providers work collaboratively with each patient. Team-based care related to cardiovascular disease prevention involves a multidisciplinary team working in collaboration to educate patients, identify risk factors for heart disease, prescribe and modify treatments, and maintain an ongoing dialog with patients about their health and healthcare. Team-based care includes doctors, nurses, pharmacist, dietitians, and community health workers. By the end of the funding cycle, the goal is that 100% of WW participants will be screened at clinics that have implemented team-based care policies. Description We are measuring the number of WW participants who are screened at clinics that have team-based care policies in place to improve cardiovascular disease outcomes. Formula The number of clients screened at clinics that have team-based care policies in place, divided by the total numbers of participants screened. Data Source The total number of participants screened comes from the Care for Yourself web-based data system managed by the University of Iowa. This data is reported to the CDC. The database also captures at what clinic the participants were screened. Data regarding team-based care policies at the clinics comes from surveys completed by the provider clinics. | Goal To increase the number of WISEWOMAN (WW) participants who engage in at least 150 minutes of physical activity per week by the end of the program year. Through health coaching sessions, WW participants are encouraged to increase their levels of physical activity. Physical activity includes participating in the Walk with Ease Program, joining a gym or dance class, or simply being more active at home (gardening, house work, etc). Description We are measuring the number of participants who increased their level of physical activity from the beginning of the program year to the end of the program year, by comparing data from the first and the second health risk assessments. Formula We are dividing the number of participants who reported engaging in at least 150 minutes of physical activity per week at the second health risk assessment by those engaging in less than 150 minutes of physical activity per week at the first health risk assessment. Data Source The Care for Yourself web-based data system managed by the University of Iowa | Goal Increase the number of WISEWOMAN (WW) participants who achieve blood pressure control by the end of the program year. Blood pressure is the pressure of blood pushing against the walls of your arteries. High blood pressure is called hypertension. Hypertension is a condition where the long-term force of the blood against the artery walls is high. Hypertension can cause serious health issues, including heart attacks and strokes. Normal blood is a <120mmHg Systolic/<80 mmHg Diasolotic. WISEWOMAN clients with hypertension participate in health coaching, where they learn the importance of healthy eating and physical activity to improve their blood pressure. They also have the option to participate in Medication Therapy Management, Home Blood Pressure Monitoring, and the YMCA's Blood Pressure Self-Monitoring Program. Description We are measuring the number of WW participants who were hypertensive at the initial screening visit and reached hypertension control by the end of the program year. Formula We are dividing the number of women who reached controlled hypertension at the end of the program (2nd health risk assessment) by the number who had hypertension at the intial screening.
