Description Measuring the number of organizations identifying high-risk patients with prediabetes or at high risk for developing type 2 diabetes and referring them to CDC-recognized Diabetes Prevention Programs (DPPs). IDPH contracts with willing employers, health systems and other populations that provide health care for a population of Iowa citizens. Formula Count of organizations that are funded by IDPH to develop and implement policy and systems changes to screen, test and refer patients with prediabetes or who are at high risk for developing type 2 diabetes. Data Source IDPH Diabetes Prevention Program | Goal Increase participation by increasing access to diabetes prevention programs (DPPs). Description The number of Iowa diabetes prevention program using the HALT platform. HALT is a virtual platform licensed by IDPH. HALT is a turnkey platform that allows the diabetes prevention program to complete the year-long cohort completely virtual. Participants can review the weekly lesson at their own leisure, upload pictures of the meals they eat, communicate to their coach and their group. Participants also upload their body weight weekly. Having access to HALT, programs are able to breakdown barriers participants often have. Barriers can include childcare, travel and work. As more and more programs choose to use HALT, access and availability to evidence-based programming increasing for the adult population in Iowa with prediabetes or high risk for type 2 diabetes. Formula Numerator: The number of HALT DPPs. Denominator: The total number of DPPs in Iowa. More people who are being identified to have prediabetes or a high risk of type 2 diabetes, more people can be referred to local programs which will decrease the incidence rate of type 2 diabetes in Iowa.
