4	Total Category Weight Enter weight for primary and secondary criteria. Note - primary criteria must be at least 75%.	Primary Criteria (Required for All Subgrantee Selection) States may add or adjust questions based on the three primary criteria listed on page 43 of the BEAD NOFO. Method to weigh, record answers, and score are up to the discretion of the state.	Weight Selected weight should be consistent across all subgrantee selection rubrics.	Determination Determinations should be consistent across all rubrics and map directly to a provided score. NTIA recommends adding answers as dropdowns.	Score NTIA recommends having score automatically populate, based on answer drop downs.	Explanation Each rubric should substantiate why an applicant received their answer/score.

5	75%	Minimal BEAD Outlay. The total amount of funding required to complete the project area in the application, accounting for both the total projected cost and the applicant's proposed match, which must, without a waiver, cover at least 25% of the project cost, with the specific points awarded increasing as the BEAD expenditure decreases for each location serviced in project area in application	50%	Total of 50 points available. 45 points available for match beyond 25% and 5 points available for cost per location. 6		Affordability. The applicant's commitment to provide 1Gbps/1Gbps symmetrical services within the project areas in the application at the most affordable price for customers.	15%	Total of 15 points available. Points awarded based on price to subscribers. 1Gbps/1Gbps symmetrical service below $70 monthly rate for 15 points. Fewer points will be awarded for prices between $70-$142.75 (For full table see Volume II Section 2.4.2)
7		Fair Labor Practices. Applicants must narratively demonstrate a record of and plans to follow federal labor and employment laws. Applicants who do not have a record of compliance with labor and employment laws can make forward looking commitments to strong labor and employment standards and protections with respect to BEAD-funded projects.	10%	Total of 10 points available. Applications that provide all the required information and certify they will comply with existing labor requirements outlined in the BEAD Notice of Funding Opportunity (NOFO) will receive 10 points in this category. Applications will receive points based on the information submitted for each element of the fair labor category. Applications that provide no response will receive 0 points. 8		Secondary Criteria (Required for All Subgrantee Selection)
9	25%	Speed to Deployment. Applications that commit to the deployment of a network and provide services to each BSL within the Project Funding Area within four years from the date of the grant agreement with UBC.	8%	Total of 8 points available. Speed to deployment will be scored on a sliding scale based on preliminary planning efforts provided within the applications, and on commitment to deploy service in fewer than four years from the time of award. 10		Local and Regional Coordination. Local and regional collaboration is required to ensure that applicants have properly engaged Utahns to determine their communities' needs.	6%	Total of 6 points available. Prospective subgrantees must demonstrate extensive community engagement and local planning. Points will be awarded for proof of formal coordination (For full table see Volume II Section 2.4.2)
11		Community Impact. A high priority of this grant is engagement with the impacted community. The Applicant is to work with the local community to identify an innovative means of providing a public benefit that addresses the community’s needs and includes as a component of the proposed project, a long-term public benefit to the impacted community.	7%	Total of 7 points available. Prospective applicant must provide evidence of community engagement and detail an innovative means of providing a public benefit that addresses the community’s needs and that includes, as a component of the proposed project, a long-term public benefit that addresses the community’s needs. Points will be awarded based on the quality of the plan to provide community impact, as well as evidence of community support and sustainability (For full table see Volume II Section 2.4.2)
12		Workforce Development and Job Quality Opportunities. Applicants will give opportunities to hire and utilize minority-owned and women owned enterprises that are considered to be qualified broadband contributors.	3%	Total of 3 points available. Points will be awarded based on documentation of engagement plan with workforce development organizations, letter of support from such an agency, and documentation of a plan to utilize local workforce (For full table see Volume II Section 2.4.2)
13		Open Access Networks.The BEAD program encourages Utah to adopt selection criteria promoting subgrantees’ provision of open access wholesale access to last-mile broadband service providers for the life of the subsidized networks, on fair, equal, and neutral terms to all potential retail providers.	1%	Total of 1 points available. Recipients that commit to offering wholesale broadband services at reasonable and nondiscriminatory rates and terms for the useful life of the network assets will receive points based on the open access details in the application.(For full table see Volume II Section 2.4.2
