Reports from local public health agencies show local engagement in statewide health improvement. Description As more local public health agencies submit a Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) and complete annual Progress Reports (PR), it shows greater understanding of the importance of planning on driving statewide health improvement. Formula Numerator: Number of counties that submit a CHA and CHIP/PR to IDPH. Denominator: Total number of Iowa counties (99). This measure shows where health improvement progress has been made and where additional efforts are needed. Description Iowa's top health-related issues are the culmination of an intensive needs assessment process that includes recommendations from individuals and private and public groups from every county as well as an analysis of health and demographic data that compares Iowa's health status with health status in other states. In the Healthy Iowans 2017-2021: Iowa's Health Improvement Plan, almost 100 organizations agreed to take action to address Iowa's top health issues. Each year, progress is tracked, plans are revised if needed, and a progress report is prepared. Criteria for assessing progress is a 5% improvement over a 5-year period in the baselines for measures of health improvement for each of Iowa's top health issues. Formula Numerator: Number of health improvement measures that show progress toward 5-year target. Denominator: Total number of health improvement measures in the current state health improvement plan. Data Source Bureau of Public Health Performance tracking records using various national data sources such as County Health Rankings, America's Health Rankings, CDC Wonder, Behavioral Risk Factor Surveillance System (BRFSS) and others. | Goal To increase the reach of public health education in the state of Iowa. Description The Healthy Iowans newsletter has over 3,000 subscribers. These subscribers include a variety of individuals with a vested interest in the health of Iowa (nonprofit organizations, businesses, state agencies, local public health departments and more). The Healthy Iowans newsletter was created to provide brief and engaging content on public health topics relevant to the state. The primary topic of each newsletter changes monthly, but examples include: the social determinants of health, Healthy People 2030, data resources, etc. By showcasing a wide range of topics, the Healthy Iowans team intends for the newsletter to be a valuable and thought-provoking resource for subscribers. Through this performance measure, the Healthy Iowans team will gain insight into how many subscribers open the newsletter after receiving it. Theoretically, the more unique subscribers that open the newsletter, the greater the newsletter's reach. Formula Numerator: Number of unique subscribers who opened the newsletter. Denominator: Number of unique subscribers who received the newsletter. Data Source Newsletter metrics provided by the Gov-Delivery system. | Goal To increase engagement among subscribers by making the newsletter more consistent, more eye-catching, and more comprehensive. Description The Healthy Iowans newsletter has over 3,000 subscribers. The primary topic of each newsletter changes monthly, but examples include: the social determinants of health, Healthy People 2030, data resources, etc. In July of 2022, the Healthy Iowans team updated the newsletter format to include two new sections titled, "Trainings and Events," and "News and Resources." Additionally, the newsletter was sent on a more consistent basis — the first Friday of every month. Through this performance measure, the Healthy Iowans team wants to assess whether the recent changes have made an impact on subscriber engagement. Formula Numerator: Number of conversions (the number of individuals who clicked on a link within the newsletter). Denominator: Number of unique subscribers who opened the newsletter.
