3 | 8/6/2015 | 6 hours | I was given the grand tour of Dr. Rosado’s work area and her partners. Dr. Rosado allowed me to shadow her and her medical assistant Lillian if the parents of the patients allowed me to do so. I was not allowed to shadow Dr. Rosado or Lillian if the patients were from the age of 11 to 17. When I was allowed to shadow Lillian, I witnessed her take the weight, height, oxygen level, temperature, pulse, and blood pressure of the patients. In some cases, Lillian would check the hearing and vision of the patients, usually when the patients were visiting the Dr. Rosado for their physicals. Afterwards, I was allowed to shadow Dr. Rosado and observe her at work. Dr. Rosado would try to have a conversation with the patients and their parents, trying to make the patients calm and relaxed before she checked them. She would check the patients’ eyes, ears, mouth, stomach, and privates. She would listen to the parents to see why the patient was visiting and then recommend medication if the patient required it or not. Dr. Rosado then left to a meeting before lunch. Once lunch was over, I was sent to highlight some forms and create more medical sheets. | 6 hours |
4 | 8/10/2015 | 5 hours | Dr. Rosado wouldn’t be in the clinic for the week, however, she had another pediatrician cover her. I shadowed this pediatrician for the week. Her name was Dr. Mary Tao. I was taught about the different types of shots required for patients at the age of eleven, one of the shots being the tetanus shot. I observed Lillian prepare the shots by gathering the needles and looking over the containers with the vaccines. She told me that before using a vaccine to always check the container to make sure the vaccine itself isn’t expired before using it. She also told me that the type of shot a tetanus shot is intramuscular, a shot that is entered in the arm at a 90 degree angle. During a patient’s physical, Dr. Tao checked the patient to make sure there were no growing delays, making recommendations for a healthy development. At the end of physical, Dr. Tao would give the patient a high-five and a sticker. During another physical, the patient was required to have a blood test. I witnessed Lillian gather the tubes and needles for the blood test. She told me that the required tubes for a blood test were colored lavender and tiger. When doing the blood test, Lillian told me that the needle enters the vein at a 15-20 degree angle, making it a Venus Puncture; where the needle goes right into the vein. | 11 hours |
5 | 8/11/2015 | 5 hours | During my time at the clinic I spent a majority of my time helping Lillian check the patients before seeing Dr. Thomas, as Dr. Tao wasn’t in today. I witnessed Lillian help two patients with their physicals. She told me that all the information she writes down on each patient must be entered into the Health Care Partners system for each separate patient. Lillian also told me her story of how she became a medical assistant for Health Care Partners. She also told me the different patterns needed for a set of three shots, which is required for protection. I was also advised that I shouldn’t throw away anything with the patient’s name or information on it without concealing it first, for it is a must. By the end of the day, I was introduced to the centrifuge. The centrifuge is used to separate the serum and plasma in someone’s blood by spinning the tiger colored tubes that contain a special medical wax that is used to help in the separation. | 16 hours |
6 | 8/13/2015 | 6 hours | Dr. Tao was back in today. I accompanied her for a 6-year olds physical. The patient appeared to be dehydrated with eczema around her ear. The patient was required to have a urine test, which would then be sent to a medical lab to determine whether the patient had an infection. Dr. Tao also recommended to the patient’s parent to use the lotion Aquapher for the patient’s ears and Eucerin for the patient’s stomach. I was also taught the 7 Rights, the 7 things that need to be followed for shots. Towards the end of the day, I was asked to create medical packets and highlight a few more medical forms. | 22 hours |
7 | 8/19/2015 | 6 hours | Dr. Rosado had returned today, and I was allowed to shadow her and Lillian again. One of the patients that Dr. Rosado attended to had a nasty cough and a slight fever. According to the parent, the patient wasn’t able to sleep well due to the cough. The patient was given a breathing treatment and sent to receive x-rays so that Dr. Rosado could give a final solution to tackle the cough. When the x-rays were done, Dr. Rosado and I examined the patient’s lungs. Dr. Rosado pointed out that the lungs had hyperinflation, a sign of early pneumonia, and a little bronchitis. Dr. Rosado recommended to the parent that the patient would receive a shot and an inhaler to help with the child’s cough and fever. Another patient Dr. Rosado attended to was behind on their shots. Lillian was in charge of preparing the shots and giving the shots to the patient. I witnessed the preparation of the shots, but this time I noticed that she was mixing water with the vaccine, and she informed me that she needed to mix live shots with water in order to make the vaccine liquid to flick the vile to make sure it mixed well. By the end of the day, Dr. Rosado informed me of her reason for becoming a pediatrician. | 28 hours |

10 | 9/15/2015 | 2 1/2 hours | I worked with both Dr. Rosado and Nurse Hernandez once again. While Dr. Rosado was filling out forms for a patient, I followed Nurse Hernandez as she looked over the stocks of vaccines. After a quick check, Nurse Hernandez came to the conclusion that three vaccines needed to be restocked and would later order the following vaccines: MMR, Gandasil, and Varicella. Afterward, I accompanied Dr. Rosado as she checked over a patient with asthma. I felt a little connection with this patient due to the fact that I, myself, have asthma as well. Dr. Rosado recommended to the patient's mother to have the patient use his inhaler 10 - 15 minutes before P.E.. Dr. Rosado then proceeded to order two inhalers for the patient as well as an aerochanmber due to the patient having no experience with an inhaler. The next patient was in for receiving allergies the week previously. After a quick check of her heart, lungs, ears, nose, and mouth, Dr. Rosado came to the conclusion that the patient's nose was stuffy, but her throat and chest were fine, coming to the conclusion of a common cold. Dr. Rosado recommended to have the patient to drink plenty of fluids and for the patient's parent to not give the patient any medicine for fever if the patient doesn't have a fever. | 2 1/2 hours |
11 | 9/16/2015 | 3 hours | Today, I would accompany Dr. Rosado as she checked over a patient with a fever and an ear ache coming from his right ear. She assigned antibiotics for the patient to take for his ear ache and told the patient's parent that the reason for the fever was simply a common cold. Dr. Rosado then informed me that before a child could return to school, they would need to fever free without medication for 24 hours. The next patient Dr. Rosado and I visited was suffering from what appeared to be an allergic reaction. Dr. Rosado speculated that the allergic reaction could've been caused by Hemoratic or Superficial skin Infection. She recommended the Rosefin Antibiotic to help with the condition and instructed to the patient's parent to have the patient's leg raised, iced, and have hydrochorizone cream be placed on the agitated skin. | 5 1/2 hours |
12 | 9/22/2015 | 3 hours | The patient that Dr. Rosado looked over for today was in for coughing up a lot of flem in the past week. Dr. Rosado checked the patient's lungs, mouth, and nose. She stated that the patient's lungs sounded clear, no wheezing in the patient's chest, therefore, no mucus in the patient's lungs. However, the patient's nose was indeed stuffy, indicating that the mucus was coming from the patient's nose. Dr. Rosado recommended to the patient's mother to have the patient take a couple of puffs from an inhaler before bed and to use Mucinex as well. The next patient was a baby. The patient was in due to constant vomiting and diarrhea. The patient didn't have a fear and according to the patient's mother, the patient had diarrhea twice a day since the last time doctor's appointment. After listening to the patient's stomach, which was somewhat gurgily, Dr. Rosado came to the assumption that the formula the patient was consuming was the cause. She recommended to the patient's mother to have the patient consume a gentle ease formula and to call if it didn't help. | 8 1/2 hours |
13 | 9/29/2015 | 3 hours | The patients that Dr. Rosado would look over first were a pair of siblings. The first sibling was in for a cough and running nose occurring since the previous week. This patient had a congested cough but no fever. Dr. Rosado listened to the patients lungs to dscover that the chest was clear, thus concluding that the patient's congestion came from the patient's nose. Dr. Rosado recommended sinous drops, chewable ibeprofan, and to have the patient's head ellevated. The second sibling had a bad cough but no fever. This patient's chest was clear and had no mucus. Basically, this patient had a common cold and was recommended Mucinex. The next patient was an asthmatic whose asthma had been acting up a few days prior to the check up. The patient had been taking treatments, yet the patient's wheezing had been the same with a tight chest. Dr. Rosado gave the patient a breathing treatment then checked the patient's chest. There were faint wheezes, and Dr. Rosado recommended to the patient's parent to have the patient receive a treatment 3 - 4 times a day with 4 hours spaced in between each treatment. | 11 1/2 hours |
14 | 10/6/2015 | 2 hrs & 45 min. | Today Dr. Rosado and I had a return visit from a set of twins as patients. They were both in for their physicals, which meant that they would each have a blood test, and they were to receive the flu vaccine as well as their 4 year old shots. Dr. Rosado instructed the patients' parent to make sure the patients were having balanced meals for a healthy diet. The next patient was also in for her physical. She would be receiving the flu vaccine as well and was up to date with her vaccinations until next year. | 14 hrs & 15 min. |
15 | 10/20/2015 | 2 hrs & 30 min. | The first patient I watched Dr. Rosado look over was a 2 year old. The child was in to receive some vaccines that are required before a child is allowed to start school. These vaccines were pentacel, the MMR, and the chickenpox vaccines. The other patient was a 2 1/2 year old. This child was in for a check-up and wouldn't receive any vaccines until they were 4 years of age. However, Dr. Rosado required that the child should receive a dental check-up and return to Dr. Rosado at three years of age for their physical. | 16 hrs & 45 min. |
16 | 11/3/2015 | 2 hrs & 40 min. | I would shadow Dr. Rosado during her examination of a 2 year old with a cold. One of the reasons for the child's condition was due to a lack of fiber. Therefore, Dr. Rosado recommended to the parents to feed their child more fruits & vegetables. The child was scheduled to have an abdominal x-ray to see how congested they were. The next patient was a 15 month old. This patient was in for 2 scheduled vaccines. The last patient I was allowed to see was a 10 year old in for their physical. 17 | 11/17/2015 | 2 hrs & 40 min. | Today's first patient was a 2 week old baby. After the check-up, Dr. Rosado explained to me that the baby had collic, trapped gas bubbles that won't pass, usually happens around same time everyday. The solution is to burp the baby during feeding. The next patient was a 9 year old, in for stomach problems and a fever. After her examination, Dr. Rosado recommended to the patient and their parent that the patient drink more liquids, no milk or yogurt, eat plenty of vegetables, and chamomile tea. The next patient had the coxsackie virus or hand foot in mouth virus, which causes blisters in hands, feet, and the mouth. | 20 hrs & 50 min. |
18 | 12/15/2015 | 1 hr & 25 min. | Today Dr. Rosado was out for the week so I shadowed Dr. Tao once again. The patient I saw while shadowing her was a few weeks old baby. Dr. Tao started off by creating a friendly conversation with the parents. Their concern was the fact that their child was eating quite a lot. Dr. Tao explained to the parents that all babies are different and will eat as much as they want. The child was growing and gaining weight at a steady pace. Dr. Tao then checked the baby’s heart, lungs, skin, ears, nose, hips, toes, and pulse. It turned out the child’s skin was dry. Dr. Tao explained that the dryness of the skin was due to the change of the baby’s environment. She recommended a humidifier to help with the dryness of the skin, and once the baby is 6 months old, to apply child lotion. Once the parents and patient left, I took my leave, as there would be no patients I could see for the rest of the day. | 22 hrs & 15 min. |
19 | 12/16/2015 | 2 hrs & 20 min. | I shadowed Dr. Tao today as well. The patient was a 4 year old with a cold. To help comfort the patient, Dr. Tao created a conversation with the patient and brought stickers. According to the patient’s mother, the patient had been sick since Sunday night with a high fever. The patient had been coughing and vomiting as well. Dr. Tao then proceeded to check the patient’s ears, nose, throat, heart, stomach, and lungs. The child’s nose was indeed congested and there was also some blood. The child’s lungs were congested as well along with a deep cough and a hurting stomach. Dr. Tao recommended a humidifier to help with the breathing as well as Vick’s to open her lungs and saline spray to get rid of the nasal congestion. She was recommended to have no milk for one week and nothing to spicy, greasy, or sweet as well as ½ 7up & ½ water every ½ and hour to help with her stomach and to eat bananas, toast, and crackers. Once Dr. Tao was finished, I took my leave after the last patient’s parent refused to have me shadow Dr. Tao while checking on the patient who was a newborn. | 24 hrs & 35 min. |
20 | 12/22/2015 | 3 hrs & 30 min. | Dr. Rosado was back today so I shadowed both her and her medical assistant Ms. Hernandez. As I shadowed Ms. Hernandez, I witnessed her take the weight, blood pressure, pulse, and temperature of 7 patients. I then joined Dr. Rosado as she went to check on her first patient, a one-month baby boy. He was in for sneezing, coughing, and watery eyes. Dr. Rosado checked his heartbeat and chest. His chest was clear, indicating that the mucus was in the back of his throat, resting on his airway. Dr. Rosado recommended little saline drops, a humidifier, and positioning him in a sitting position. Once finished, Dr. Rosado told me than when a patient is less than 2 months old, they must always be brought to the doctor’s even if they might sound find based off the parent’s information over the phone, it’s better to be safe than sorry, considering they can potentially have serious infection. | 28 hrs & 5 min. |
21 | 12/29/2015 | 4 hrs & 15 min. | In the beginning I wasn't allowed to see any patients due to all of them being adolescents. However, I did witness an I.V. be prepared for a patient suffering from dehydration. When I was finally allowed to shadow Dr. Rosado once again, the first patient I saw was 2 week old who was in for a check-up. After this day's check-up, the patient would not receive any other immunizations until the patient is 2 months old. The next patient I would see was 6 month old who was in due to a lot of coughing and flem. After her examination, Dr. Rosado recommended to the patient's mother that they should elevate the patient's head and use clarinex, which is baby clariten. The last patient I saw for the day was a 2 year old. The patient can now drink whole milk, eat cheese, and eat yogurt. The next time the patient woll come in will be to receive their 3 year and 4 year shots for starting school. | 32 hrs & 20 min. |
22 | 1/5/2016 | 2 hrs & 50 min. | Today I was informed that the power was out for maintenance apparently continuing from last night. Many appointments were cancelled until the power came back up at 8:00 a.m. After I was informed of what transpired, I was given permission to shadow Dr. Rosado as she observed a 4 month old. According to Dr. Rosado, the patient was ready to eat food, and she recommended the patient's mother to start with rice cereal, 2 tablespoons in the morning & afternoon. After coming to the conclusion that the patient had nothing more than a congested nose, Dr. Rosado recommended a humidifier. Dr. Rosado then stated that the next time the patient would need to come back would be in 2 months for their 6 month immunizations. The next patient I was allowed to see was a 4 year old. This patient was in for receiving a bump on their leg. It turned out to be nothing more than a bruise. Dr. Rosado recommended to put ice on the bruise for 15 minutes. 23 | 1/6/2016 | 1 hr & 30 min. | Today was my grandmother's birthday so I couldn't stay as long as usual. Therefore, I was tasked to create copies for Dr. Rosado. 24 | 1/12/2016 | 2 hrs & 20 min. | On this day, the first patient I saw was a 4 year old. Apparently he had a muscle weakness in his right cheek, stuffy nose, tear, and red eyes. After giving a full examination, Dr. Rosado recommended 5 ml of clariten each night for allergies, an eye test he would receive today, and based off the test, a possible recommendation to visit the optometrist. The results from his test proved that he didn't need to visit the optometrist. Instead, Dr. Rosado recommended Natural Tears as a method to lubricate his eyes. The last patient I saw was a 4 month old. This patient was in for his second set of immunizations. After the examination, Dr. Rosado noticed that his legs were somewhat dry, so she recommended to the patient's parent to give him Aveeno for Babies lotion. | 39 hours |

According to the patient's mother, the patient had hit their head against the floor on Sunday, and had a little bloody nose on Monday. Dr. Rosado listened carefully to the patient's parent as she observed the patient. Once her examination was over, Dr. Rosado concluded that the patient was fine and recommended saline water to help clear the patient's nostrils. Once the patient had left, Dr. Rosado explained to me that when patients are this young, and they suffer a head injury, it's important to for unconsciousness or signs of wanting to sleep. | 47 hrs & 25 min. |

29 | 2/24/2016 | 2 hrs & 31 min. | I was able to shadow Dr. Rosado today. One patient was in for a few symptoms such as a headache, fever, and bodyaches. According to the parent of the patient, the patient had been sick since Monday, however, the patient did not vomit or had any diarrhea. After her examination of the patient, Dr. Rosado had come to the conclusion that the patient was developing a flu. Her recommendations to combat the illness included a humidifier, warmer drinks, and some sleep. The next patient I was allowed to see was in for a cough and an ear infection in their right ear. Dr. Rosado checked the patient's lungs, ears, throat, and nostrils and came to the conclusion that the main reason for the patient's cough was the patient's stuffy nose. Dr. Rosado then assigned antibiotics for the ear infection, recommended a humidifier for the cough, no milk for a couple of days, plenty of water, and honey-cough syrup. 30 | 3/1/2016 | 2 hrs & 36 min. | The patient I saw today was 5 months old. They were in for eczema and a white spot on their upper mouth. Dr. Rosado maintained eye contact with the parent of the patient as they explained the reason for making an appointment. After Dr. Rosado observed the white spot, she informed the parent that the white spot was a pearl. A pearl is a temporary tooth that appears when a child begins teething. Dr. Rosado checked the patient's heart, lungs, ears, and nose. She noticed that the patient was wheezing, and their nasal passage was swollen. The patient received a breathing treatment before leaving. The 3 year old was in for a physical. Dr. Rosado checked to see how the patient was growing, including checking the patient's back, neck, lungs, eyes, ears, nose, and throat. Dr. Rosado concluded that I most likely had a sinous infection and recommended a decongestant for me. She concluded that both patients would need to receive blood work, however, neither would need to immunizations until they reached the age of eleven years. Once the appointment was over, Dr. Rosado informed me that a child's mind can decide how they receive their shots, with or without pain. 34 | 4/13/2016 | 2 hrs & 50 min. | I was allowed to see a 6 month old, a 3 year old, and a 7 month old. The 6 month old was in for their third set of immunizations. Dr. Rosado was informed of how well the patient was developing from the parents. Dr. Rosado informed the parents that the patient would begin to stand between the ages of 9 months and 1 year. The patient would receive fluoride vitamins to help with the patient's teething and gums. The 3 year old was in for pain in their left ear since Monday, having cold symptoms on Sunday, and having their right eye discharge on Sunday as well. Dr. Rosado examined the patient's heart, lungs, ears, and throat. The patient did indeed have an ear infection, with their ear being red and inflamed. The patient would receive antibiotics for the ear infection. The 7 month old patient was in due to having an allergic reaction although they hadn't been eating differently. Dr. Rosado examined the patient but found no signs of a fever. The patient did have though dry skin. Dr. Rosado recommended to apply child Aveeno lotion for the patient's skin. She also recommended for the patient to take Benedryl to help with the patient's allergies, and for the patient to not eat anything seasoned. | 66 hrs & 5 min. |

42 | 1/16/2016 | 4 hrs | Today I managed to complete the bases needed for the final construction of my scale model of the Health Care Partners Clinic, composed entirely out of Legos. Now that the bases have been constructed, I can begin my sketches for the final construction based off of my materials. There has never been a Lego Health Care clinic, so I am going to have to think hard of where to place each Lego to receive an accurate description of my place of mentorship. | 4 hours |
43 | 1/22/2016 | 4 hrs | I sketched out a design of my healthcare clinic that I will produce from the materials that I have on the way home from school. As soon as I got home, I gathered my materials and tore apart my prototype to shape it into the model I had designed. There were a few struggles I had as I tore apart my prototype and began to form my model. Some of my materials fell apart, which resulted with me taking time to reform them. I managed to gather more materials from my little brother, using some of his Lego bases to construct a bottom and second floor. The second floor is coming out smoothly with an elevator, the makings of a hallway, a door that leads to the pediatric center at Health Care Partners, the two separate waitng rooms, the preperation room, Dr. Rosado's office, and two of the three rooms patients wait to be seen by Dr. Rosado. | 8 hours |
44 | 1/23/2016 | 4 hrs | I began the construction of the pillars to help support the first floor of my Lego pediatric clinic. In order to do so I had search through the countless Lego materials my brother contains. The materials required for the support beams were Legos that had the capability to form a small square base. So far, the materials I was able to acquire were just enough to construct two support beams out of six that will be needed to complete the component overall. Besides constructing the two support beams, I managed to connect the three bottom platforms, where the support beams are being constructed, together. | 12 hrs |
45 | 1/25/2016 | 6 1/2 hrs | I tell ya, it was a crazy day today. I arrived to my work station to discover that some of my Legos had fallen apart. So the first thing I did was repair what was broken. I then got to work constructing the rest of the support beams to hold up the second floor. I managed to complete the original six support beams, and then I decided to test how the beams handled the weight of the upper floor. Unfortunately, the result was not one I had hoped for. The six support beams were not enough to hold up the upper floors. Therefore, I gathered more materials that I could find within my household to create two more support beams. Once all the support beams were completed, I decided to create the entrance of the Health Care Partners Clinic. I was able to construct the sliding doors of the clinic's entrance, however, it doesn't actually slide, as I don't have the materials to actually cause the door to do that. Overall I say today was a productive day. The bottom floor is completed, so all that's left is the upper floor and attaching the top floor to the bottom. | 18 hrs & 30 min. |
46 | 1/29/2016 | 3 1/2 hrs | I noticed I was lacking the resources I needed, therefore, I decided to head out with my father to purchase more Legos for the rest of the model. Once we had arrived back home from our trip to Target, I set back to work on the model. I managed to make the last two waiting rooms for patients who are ready to see the doctor. I finished the doctor's office and an office for one of the employees. I also created door ways to each room and office as well. | 22 hrs |
47 | 1/30/2016 | 7 hrs | I got up early today and looked over my model. It appeared that I had covered most of the main areas of my workspace but not all of it. Therefore, I came to the conclusion to create the other areas of my workspace. I created the work area where I usually await for Dr. Rosado before she gives me the 'ok' to see a patient. I even added the computer Nurse Hernandez usually uses. I added bathrooms as well in the sections of where they're located. I created three of them in total. Furthermore, I added chairs to the waiting rooms & offices, an area where medical assistants & nurses answer patients' phone calls, the medical beds patients sit on during their appointment, and the wall & door way that separates the waiting room from the offices. | 29 hrs |

I found one article extremely interesting, as it provided lots of information on how address patients with different backgrounds. It presented information that should executed with all patients, and it even had some tips on how to address specific patients. I also visited the website I'm constructing my presentation on and learned some of the basics behind how to operate the mechanics of the presentation itself. The method of this presentation style worked by recording my voice and using the audio to present my presentation. I will then add visual effects to progress the presentation, having it come to life with both visuals and audio. I then logged back into my account to begin establishing the presentation itself. 56 | 4/20/2016 | 7 hrs | Today I focused on completing at least half if not all of my presentation slides. I mainly focused on completing all of the recordings per slide, which was no easy task. My recordings were created from the website's personal voice recorder that was supplied for my presentation. It was more difficult than I originally thought as the microphone on my computer wasn't exactly the best. It took me two to three tries to record my lines per slide before I settled on a recording. Once my recordings were completed, I began to establish some visuals that matched the audio as close as possible, which again was not so simple as I had limited resources for my visuals. 57 | 4/21/2016 | 7 hrs | I finished the remaining amount of slides for my presentation. However, even after I had completed it, there were still some issues with my presentation. I conducted a preview of my presentation only to be resulted with a video with mismatched audio and visuals. Some of my visuals were placed on the wrong slides with a different audio recording that was meant for another visual effect. I went back to my work and managed to eventually fix the mistaks I made earlier. I conducted yet another trial run before publishing my video for public view. Overall, I'd say it came out pretty well, especially considering it was my first time using this type of software to construct a presentation. | 30 hrs |
