1. Complete the required applicant information on the Cover Sheet. Feel free to rename tab to i.e. "Org 1 - Scouts". The "Total Hours" will automatically update when information is inserted into the "Org" tabs.

2. Utilize the "Org" tabs in this workbook to record your service activities. Use one tab/sheet per organization you served (i.e. Org 1 = Scouts, Org 2 = Booster, Org 3 = PCSC, Org 4 = Animal Shelter, etc.). Complete the top portion of each tab before entering activities. The "Total Hours" will automatically calculate based information provided in the chart below. Please include the contact information for your direct volunteer supervisor or the supervisor signing off on the hours in your verification letter.

3. Each activity should be recorded within its own row on the "Org" tabs.

 -- Column A ("Date"):

 List the date on which the service activity took place. You must list each individual date and number of hours that you volunteered. Do not group date ranges (i.e. 4/1-4/30: 20 hours).

 It is best to keep this log as you volunteer rather than trying to recall all dates after the fact. Dates will automatically populate in the 00/00/0000 format and should remain that way.

 -- Column B ("Describe What You Did"):

 Explain the volunteer activity in full sentence(s) (i.e. I volunteered at Meals on Wheels, delivering meals to homebound clients.).

 -- Column C ("Hours"):

 List the number of hours you volunteered for this specific activity. Each activity can include intervals using their decimal format (i.e. 2.25 = 2 hours and 15 minutes, 2.5 = 2 hours and 30 mins, 2.75 = 2 hours and 45 minutes, etc.). Decimal format is the only acceptable format for this to ensure total hours are calculated correctly on the tops of the "Org" and "Cover Shhet" tabs.

4. Note that only hours volunteered within the KPI PVSA Service Year are eligible. KPI's PVSA service year begins August 1 and ends July 31 of the following year. For example, if you are applying for an award in 2025, only hours volunteered between August 1, 2024, and July 31, 2025, will count. Therefore, if you volunteered 5 hours on July 30, 2024, and 3 hours on August 5, 2024, only the 3 hours from August 5 will count towards the 2025 Service Year.
