Include a copy of the Private Placement Memorandum (PPM) and Subscription Agreement from the Sponsor.

Obtain a copy of the most recent Audit Letter or if not audited yet an Audit Engagement Letter. The auditing firm must be registered under Public Company Accounting Oversight Board, PCAOB (https://pcaobus.org/Registration/Firms/Pages/Registe redFirms.aspx).

Submit these to a Fidelity representative who can forward to the AIP back office (the back office is not supposed to interact with retail customers, but they do interact with Sponsors and institution customers). Another easy way is to drop the paperwork to a local Fidelity office. It typically takes 15 days to three months.

At this point, you can sign the Subscription Agreement, send to the Sponsor and a copy to Fidelity (again through a representative, or drop it in a local office). Also you need two other forms: (a) Request for Transaction (Letter of Instruction or LOI: https://drive.google.com/file/d/0Bw1oBlzDITrzZUhLM1B1WnppRU0/view?usp=sharing) and (b) Alternative Investments Addendum and Custody Agreement (https://drive.google.com/file/d/0Bw1oBlzDITrzWU8tQ0hITTFBRkE/view?usp=sharing).

Once the asset review is complete, the AIP will send some final agreement to the Sponsor to agree the fund hosted under Fidelity’s AIP.

Once they receive the final paperwork, it takes them a day or two to approve and wire the money to the Sponsor. The security should be shown at this point.

Note for submitters: Although in the asset review form it says you can email the forms to the AIP, you cannot email them by yourself. But you can have the forms emailed by the Sponsor.
