In some states, CHIP covers pregnant women. She also provides support and individual and group education to discharged patients and community members.  		Access to Care is a non-profit organization providing primary care to low-income, uninsured, and under insured residents of suburban Cook County and northwest Chicago. Welcoming all residents regardless of their race, age, religion, country of origin, gender identity, orientation, physical and mental ability, or immigration status.
Access to Care Helps:
• Adults without health insurance.
• Adults with health insurance but who pay a high deductible of $1,500 or more.
• Adults who are otherwise ineligible for public health programs, such as Medicare and Medicaid.
Members can get a 90-day Rx supply for the cost of the 30-day supply – additional financial assistance to cover monthly Rx costs.
Fee diabetes strip tests. Mental health services – members get 8 sessions for just $8 each. Community Resources - that can help you with food, dental, vision, daycare, and more. For assistance with transportation call 312-864-7433. Covers expensive meds.
