General Instruction to fill this observation solution template

Recommended: We suggest you use a unique resource name to avoid duplication in reports.

Don't change any labels of the tab and Don't change any column name's in each sheet

Take the help of the 1st row in each tab with 'Description of each field' as mentioned in below to understand what needs to be filled in a particular column

min_number_value : score for this slider value , min_number_value + 1 : score for this slider value , ...... , max_number-value : score for this slider value.

Ex : if min_number_value = 1 , max_number_value = 5 .
Format :- 1:0.5,2:1.5,3:2.5,4:3.5,5:4.5
