Met with Rupta (regional manager for neurostar), Michelle (marketing from neurogstar) and Lissette (clinical coordinator for success TMS). Typically there is less than 1 week wait to get an intake w/ a clinical coordinator. Sessions are 7 weeks, 19 mins, 5 days per week. Clinic is open 7am-7pm. Appts are 30 mins each. New psychiatrist coming in August. Currently it is a NP that "pops in" every 7-10 sessions. There are also 3 post follow ups. They have 4 machines. Currently get insurance coverage for TRD, and Cigna pays for OCD. Private pay is $8600. Success TMS is merging w/ "green brook". Best wayto place a referral is for me to email the face sheet to Lissette and she will call the patient to schedule. They don't allow patients to be on 450mg of bupropion. Also don't treat those w/ alcohol use d/o. The patient needs to be able to hold off on their benzo on the day of the brain mapping. I told Lissette I would email her to schedule a time for me to tour the office.

A lot of family programming and holistic treatment like accupuncture, phsycail therapy, exercise, yoga, swim, meditation. Customized for anything. Creyos is a web based, self assessment tool. The scores are compared to 1000s of other people at the same age. The tool was developed by neuroscientists. Tests domains such as memory, concentration, reasoning and verbal ability. Test is taken on computer or tablet, takes about 45 mins or less. The patient can use practice rounds first. If the person gets a question correct, the task will get harder and harder. This test was initially built for geriatric research. The changes will be charted over time. There is a mini test that is 24 mins for ADHD. There are staff scientists that can help interpret. For malingering, there is a “potentially invalid result” that describes abnormal behavior. Looks at things like reaction time and if they are clicking the same button over and over. For the lite plan, this is for 75 patients, costs $400 a month. Might get 25% discount. Plan resets every one year. 2 week free trial. Pay in 4 installments. Make a 12 month agreement. The code for reimbursement is 96136 if the test is administered in the office only. For my interpretation, that is a $134 reimubresemnt and its an add on code of 96132. I can test 5 people on the sample. Website is health.creyos.com. My passwork is Creyos12345!. For the BDNF, this is the brain fertilizer. The met means there is reduced secretion of BDNF. On the page where all the meds are listed, the bolded things mean that there is a variation in the gene. If its not bolded, that means its normal. The down arrow means that there are lower levels of the medicine, bc the metabolism is faster. The yellow means its moderate risk and the red is high and green is low risk. The CPIC is just the “consortium” for which this data is gathered. On the genomind website, there’s also an interaction checker where you can check other meds.

Met w/ Dave. Lifeskills has residential (private pay) in Deerfiled, that is about 60-90 days. They do general mental health, and also substance. They are in process of adding an affiliation w/ a hospital in Fort Lauderdale for more acute cases (ppl who can’t participate in therapy). They have a PHP that is from 10-4, 5 days per week in Delray. This inclues groups, individual therapist, yoga, dietician, exercise. Fun things include karaoke, and a full music, recording studio. Blue Cross and United are in network, everything else is private pay. I went to the building where php/iop is located. Parking was a bit tough. There was a diverse patient population present. There is a IOP that is 3x per week in the mornings that is ?private pay. They have “transistional housing”. It’s single family homes, where you will have 1-2 rommates of the same gender. It’s not just sober housing. They provide grocery budget and transportation to appts. They check on you q1 hour. It’s about $5500 for the transitional housing per month.

I met virtually w/ Sarea Foley, the clinical outreach coordinator for Anxiety Institute. She is from Iowa, has 3 yr old daughter. They have clinics in Connecticut and Virginia, but just expanded into Boca in July 2023. They have 3 levels of care. The “intensive day program” is 5 days per week, approx 4 hours per day. It’s about $30-50,000. You get 2 hours per day of group therapy from 11am-1pm, and 2 hours of individual. One is private therapy and the other is exposure therapy. They primarily focus on anxiety and OCD. This track runs about 6-8 weeks. There is also the “customized IOP” that is from 4-20 hours per week. This is about 6-8 weeks. The lowest level of care is the 1:1 services. This is about 3-6 months. This is $250 per hour. They do not have a prescriber, but they do have a psychiatrist who offers consultations. Their target age range is 13-25, but they sometimes go outside of this range. Right now, all the patients are together, as it is mostly adolescents. The process starts w/ a 5 hour assessment. They are private pay, but offer superbill and sliding scale. They have an insurance coordinator to assist the families. They will work w/ autism spectrum, but no aggression or psychosis. I then went and toured the facility the following day and met the owner, Dr. Blaise. I then went and toured the faiclity and met w/ Dr. Blaise. He was very welcoming and engaging. The center was in a beautiful bulding. It was a bright, modern, beautiful space. They offer art therapy and yoga. The decor was very bright, would be very appealing to teens or perhaps a college student.
