Height (cm)160HPI40's M, hx of untreated diabetes and EtOH abuse, admitted to ICU-X for DKA in the setting of suspected COVID-19 disease. Per ED chart, pt complained of 5 days of nausea and vomiting with red streaks. Presented to the ED due to cough and SOB today without fever, chills. He was tachypneic, tachycardic, but satting 99% on RA. He was found to be in DKA (HAGMA with elevated BGL, bHydroxybuterate pending) which explains (at least partially) his respiratory presentation. CT chest showed bilateral patchy airspace disease, worse on R, COVID vs bacterial PNA. Was given ceftriaxone azithro. Borderline lymphopenia, procalcitonin low, elevated COVID labs. EKG without ischemic change but QTc >500. No transaminitis.
