21	Feb 2022	Achieved a Quality By-Name List and set chronic baseline as Jan 2022
22	Oct 2023	Have reached 10% chronic reduction milestone after having three consecutive months at 10% or more below their baseline (Aug, Sep, Oct). However, it was identified that this reduction is largely due to high moved to inactive (loss of client contact) for which they are currently working to improve data through more timely updates. Once this data improvement has been completed, they will be considered for confirmation of the reduction.
