29	3. Prospective Bidders/Supplies are required this official quotation form in the accomplishment of their quotations complete with brand for listed items
30	4. Any specification other than those required/stated in this form shall not be considered in the evaluation of quotations
31	5. Quotations must include all kinds of taxes for the item/s listed hereunder, including delivery charges
32	6. Price Validity shall be a period of Sixty(60) calendar days from the deadline of submission of quotations
33	7. Quotations shall be submitted to the Bids and Awards Committee Office, 2F Provincial Capitol Building, Brgy. III, Vigan City, Ilocos Sur

38	12. The Provincial Government of Ilocos Sur reserves the right to accept or reject any quotation and to annul
39	the procurement process or reject all bids at any time prior to contract award, without thereby ncurring any liability to the affected bidder/s.
