PLEASE DOWNLOAD this form (File > Download > Microsoft Excel (.xlsx)) before filling it in. 2. This form must be completed by the student's home institution. Individual applications will NOT be processed. 3. Columns C-R MUST be completed, or this application form will be considered invalid and will NOT be processed. 4. Students’ names should be entered into this form as they wish for them to appear on their program certificate. Special requests for name changes cannot be made after application. Exceptions will only be made for misspelled names. 5. A maximum of 5 students are allowed to apply from one institution. In principle, Japanese institutions may nominate up to three students.
