2 | Welcome to The MILL Equipment Reservation Form for FALL 2022! Fall 2022 Operation Hours: M – F, 10 am – 5 pm Please review our Health & Safety Policies before reserving equipment. To reserve equipment: - Check our Equipment Status List for equipment availability. - For the date and time you wish to operate, type your FULL NAME in the cell corresponding to the piece(s) of equipment you would like to use. - For training, please contact the respective TO/CTO. - For groups, only 1 member needs to fill their name in the reservation spreadsheet, but please include your party size afterwards. (e.g., Anush Singhal [6] ) - You may reserve equipment up to a week in advance, and avoid reserving equipment less than hour before use. - Please contact the COO first if you plan to reserve a single piece of equipment for over 4 hours at a time. * Equipment requiring supervision from trained officer | ||||
