Instructions for Implementation Plan & Budget:

1. Select Priority Funding Activities (PFAs) from the following five (5) areas: Medi-Cal Enrollment, Service Delivery Infrastructure and Capacity Building, Data Collection and Documentation, Billing Infrastructure, and Collective Impact. You can select as many PFAs as necessary.
2. Identify anticipated deliverables under the areas where you have selected PFAs .
3. Idenitfy the budget for each of the areas where you have identified anticipated deliverables.
4. Identify you administrative costs which can be up to 10% of your county's funds.
5. Identify other expenditures that your COE deems necessary. Please note that DHCS and the grant administrator(s) will be required to approve other uses of funding.

Instructions for LEA Allocations Sheet:

1. List the participating LEAs
a. Select your COE in the filter from the "List of LEAs by COE" tab
b. Copy/paste the list of LEAs into the "LEA Allocations" tab
2. Identify the funds allocated for each LEA.
3. Provide a rationale for the fund allocation for each LEA.
4. If an LEA has opted out of the Capacity Grants and has chosen not to receive funds, place 0 in the LEA Allocation Column (Column B) and provide a rationale. COEs should provide documentation indicate the rationale from LEA and sufficient outreach and engagement from COE to LEA.
5. If an LEA has opted into the countywide strategy in lieu of receiving funds, place 0 in the LEA Allocation Column (Column B) and provide a rationale. COEs should have documentation of LEAs consenting to their funding going tot he COE (i.e. signed letter).
