Thank you for choosing nhc. The Carrier Access Questionnaire is intended to provide our team with the information necessary to successfully provision your Access circuits. We would like to have as much information as possible prior to kicking things off with our underlying providers. The examples below are provided to help guide you through the respective circuit type tabs. If you have an IT vendor or internal IT resource, it is best to have them review this form and provide the requested information. Your nhc Project Manager is also a valuable resource and can assist with any questions you may have.

The "Broadband - GPON - Satellite" tab contains the required information to order your broadband service. In most cases the carrier will be providing a modem, ONT or routing device at the time of installation.

The "Ethernet - DIA" tab contains the required information to order your dedicated internet service. It is important that we cover carrier installation locations, handoffs and potential circuit extensions with this service. Existing network connections, cable riser systems and distance limitations from the carrier DMARC all pay an important role in a successful service delivery. Additional information may be required for any BGP deployments.

The "Ethernet - Point to Point" tab contains the required information to order your private P2P circuit. It is important to note that we'll be collecting DMARC and LCON information for 'both' ends of the point to point circuit and this tab represents one-half of the required information.

The "WAN-LAN" tab contains an overview of how a nhc Managed DIA Circuit is delivered and the need for hardware capable of handling a routed LAN subnet through the advertised carrier WAN network. A Managed Edge Device from nhc cares for this requirement and if included in your order will be provisioned and installed by your Project Manager.

Please note any non-applicable tabs may have been removed to avoid any unnecessary confusion. A portion of the appropriate tabs are completed based off of details gathered from your executed nhc Service Addendum. Please double check these fields for accuracy.

IP ConfigurationIn most carrier cases we can provision circuits with either Static public IP assignments or with a Dynamic allocation. NOTE: IP Justification Forms may be required for subnets requested larger than /29 (5 usable IPs).

nhc Managed Edge DeviceYour access order may or may not include a nhc Managed Edge Device depending on service type, delivery and pre-Sales Engineering involvement. Please review IP routing requirements with your nhc Project Manager if there are any concerns with required hardware, connections or handoffs.

DMARC / MPOEMain Building DMARC / MPOE are standard installation locations for Ethernet - DIA circuits to be delivered. Please plan on being able to provide access at the time of site survey and circuit installations if these are secured areas.

Shipping / Additional ConsiderationsThe prices and charges set forth in the SOA do not include taxes, fees & surcharges (if applicable), any additional equipment, cabling or pre-installation work, added features (including unknown features on assumed services), shipping, optional features, or services that may have been added at the customer's request prior to installation and billing.

CancellationPlease be aware there could be cancellation / ETF fees if the order is cancelled post submission to the carrier, once site surveys are completed or FOC date is received pursuant to
Section 8 of our Terms of Service:


Please be advised that billing could commence if the carrier is unable to complete the installation on the FOC date due to a Customer Not Ready (CNR) condition.

Additionally, cancellation fees may be imposed for a Customer Not Ready (CNR) condition including but not limited to (customer unavailable for site survey, customer non responsive, customer denies site access, customer site requirements incomplete or customer requested delay.)
