20	**Due to unforseen circumstances dates, times and locations may change for these fields trips throughout the year. Parents will be notified of specific dates of the trips, and any changes via email and the choir website.

22	*** Because you sign this form does not mean that your student automatically goes on these trips. Trip participation is based on availability, eligibility and what group they are in.
23	****Eligibility requirements for extra-curricular activities are stated in the student handbook. Students must have a minimum 2.0 GPA and not more than on F or U.
24	*****By signing this form you understand that performances are videotaped and photographed so that students have a permanent record of their work for their portfolios and that news media, district media, etc, sometimes feature our performances in stories which may include photographs or video clips.
