Please Note: For the spreadsheet to work correctly please download as an Excel file to your local computer. Go to 'File' menu at the top left, select 'Download', and select Microsoft Excel. Save to your location of choice. Open the file and after the file opens, and if necessary, click the 'Enable Editing' button at the top so that you may proceed with editing the list.

Instructions: Use the separate worksheets to enter data about residents and staff (including employees, students, interns, etc.) in your facility with respiratory illness. We recommend that the log is updated daily. There are drop down lists to help with the data entry process.
