6	Hersh-White	Jaime	if someone passes away- who is responsible for tenant portion and how will that be payment be made?	In Illinois, upon a tenants death, their estate (next of kin) is generally responsible for the tenant portion until the lease is terminated. A lease termination should be requested immmediately with a plan to vacate the property as soon as possible. If there is a reppayee involved, the rent should be paid. The landlord can use the security deposit for unpaid tenant portion. The subsidy administrator will pay through lease termination.	IDHS
7	Boland	Lindsay	what if a class members rent has been over FMR previously, does this affect lease renewal in the future once they are done with CCMTP program?	Bridge Subsidy Administrators will reach out to IDHS in the event of rent increases beyond the initial IDHS approval. Continued approval is managed on a case-by-case basis.	IDHS
8	Hilsenhoff	Erin	Given that there will no longer be a subsistence allowance, will Primes be able to close clients without income?	Class Members have the right to transition with or without income. Prime Agencies are not able to just "close" Class Members in the WebApp. There are procedures to follow for Class Members waiting to obtain income, unable to obtain income and refusing to work towards income. Please see policies P3.02 Documenting Pre-Transition Engagement and Status Changes and P3.03 CAST.	IDHS
9	hamilton	lekesha	Which is true, if someone does not have income, they will not get monthly help	Subsistence Allowance will be sunsetting in the near future. There are other community resources available for Class Members and non-Class Members in need of financial help in the community.	IDHS

A reminder that this program has run without Subsistence in the past. This was a polciy added within the last 3 years that will end in the near future. Bridge Subsidy eligibility ishandled on a case by case basis. See policy P4.08 Bridge Subsidy for Participants with No Income .	IDHS

Please work with the Housing Coordination Team andour partners at CSH who are available to help navigate this process. 24	Watson	Tynisa	sorry if I missed it but is maximus getting initial consent to from member and guardian to do outreach and assessments? then us as primes can do a informed consent essentially to say hey you are confirming working with us as your prime for the ccmtp. But maximus should also be getting initial consent especially from guardians to even do outreach and assessment, is that correct? sorry if I missed if this was covered	Maximus will not get consent forms signed for the CCMTP. Only CCMTP staff will complete CCMTP forms.	IDHS

26	Watson	Tynisa	for clarity you are stating once Maximus send a referral we have 1 day to claim them ? that can be hard depending on how many maximus sends and the frequency. Us as primes will have to claim a member as admin on assessment pro and then assign it to the care manager so that should be considered and or added on to assessment pro ( that us who claim wont be the ones doing the assessments).	Acknowleging and claiming referrals is a separate process from assigning a care manager And doing the CSP. At the moment, is expected that all Prime Agencies assign dedicated staff to claim referrals. We will be happy to address any difficulty with this process in the future since we are unable to predict Maximus referral frequency	IDHS

Transition service plans are finalized 30-60 days preceding the transition. And all CSPs are to be updated minimally, every 180 days.	IDHS
29	Hersh-White	Jaime	What is difference between Cluster Housing and CILA?	These are two different IDHS grant funded programs and they are overseen by 2 different divisions at DHS. The Clustered Housing Program is meant to be a step down for Class Members and Front Door Diversion participants. CILAs are for all people in Illinois living with mental illness or developmental disabilities. These are open to non-Class Members.	IDHS
30	Savage-Brown	Cynthia	can we consider creating a briefer format for the termination service plan, e.g. just the individualized plan section?	At this time, the termination service plan is a CSP update and is a requirement.	IDHS

IDHS is happy to provide assistance with this process in your JOC meetings when discussing projections and the pipeline. This is not a part of the Maximus assessment. Outreach and Assessment services are not a part of the transition program and Maximus does not offer transition services.	IDHS

Please see policy P5.06 Post-Transition Closure to Services, The timeline depends on when a complete 5.7 is submitted by the Prime Agency. Prime Agencies will be notified by email of 5.7 Post Transition Closure Checklist review outcomes within 5 business days of submission.	IDHS

36	Savage-Brown	Cynthia	if we don't know someone was involuntarily discharged until we get the list, 5 business days is not a lot to time to figure out whether we can locate the class member and determine if they want services, I think	It is important that PrimeAgencies reach out to Class Members who are being involuntarily dicharged as soon as possible. Otherwise, they may be discharged and you will be unable to find them in the community.	IDHS
