INSTRUCTIONS TO USE THIS TOOL

1. Go to File, Make a Copy, and save this file to YOUR google drive

If you input your information in this file, it will be seen by everyone and not saved for your own historical comparison purposes.

2. Insert YOUR information in fields/cells colored this yellow. Column C in the next tab "template" will be your initial assessment. Everything else will automatically calculate

3. Summary Tab you'll input your first and last name in cell A1 and put the date of your initial assessment in cell B1.

For follow-up assessments, you'll simply update the date on the Summary tab for Day 30, Day 90, and Day 180.

Then you'll go to the "Template" tab and input "Your Answer" for the column specific to Day 30 (column F), Day 90 (column H), or Day 180 (columm J).

Your score will automatically populate and the Summary tab will automatically be updated and you can see the severity at the bottom of the screen.

MAKE SURE YOU EMAIL this file once your initital assessment is completed and each time you re-assess so that we have a copy and can provide additional help or guidance when needed.
