Hi all! This is a handy little grad application layout that I used when applying to schools. Feel free to copy and/or repurpose this for your own needs if you'd like! You could also use this for fellowships and other types of applications as well! List of all the names of the school you're applying to goes here!I found it helpful to jot down the locations of each school so I could get a better sense of where I might potentially be living This was more helpful in interpreting my own GRE score and what I should be aiming for. All of this had to come from the school itself (rather than a message board/forum) otherwise I left it blankWhen the application is due!This took a bit of time, but each school has their own individual requirements for fee waivers and sometimes requesting a fee waiver is due before the actual application is due, so it was helpful to keep track of that hereHere I mostly put any general formatting or page limits the school had Pretty much the same info here as the Personal Statement. Although one school I came across wanted 2 writing samples, so info like that would also go hereMost schools I applied to wanted 3, but if it was any different I put that here and also who I asked to write me a letterThis is where things like Diversity Statements, hard copies of transcripts, official GRE scores, and other documents the school wants would goThis was for keeping track of any additional short answer questions the application had such as listing honors/awards or research experienceHere I listed 3 - 4 faculty members for each school that I wanted to work with and mentioned in my Personal StatementThis is where I listed any on-campus centers that I was interested in getting involved with. You can mention these in your Personal Statement as well!I just copied and pasted the link to the actual application portal here for convenience Some schools want official score reports directly from ETS, so I kept track of if I did that here. Also remember to keep your receipts if you do have to do this! Because if something gets lost in the mail, you don't wanna pay to send this againPretty much the same as sending GRE scores except with official/hard copies of transcriptsSometimes programs will ask you to list out specific specializations you're interested in.
