3	President Name, Email & Student Number:
4	Treasurer Name, Email & Student Number:
5	Bank Branch:	All clubs should have a Scotiabank account. Please provide the name of the Scotiabank branch where your account is held.
6	Bank Institution Number	Provide the three-digit Institution Number for your bank. For Scotiabank, the Institution Number is 002.
7	Transit Number	Enter the 5-digit transit number for your bank branch. The transit number identifies the specific branch where you opened your account. For example, 00406 is the transit number for the Scotiabank branch at Carleton University.
8	Bank Account Number:	Enter the account number associated with your club’s account. The account number (12 digits) identifies your individual account.
9	Starting Balance - May 1, 2024	Enter the balance of your club's account as of May 1, 2024. This should match the closing balance from the previous year's year-end ledger to ensure a seamless financial record. If you're unsure of the starting balance, please refer to your most recent bank statement as of May 1, 2024.
10	Closing Balance - May 1, 2025	The last entry should reflect your actual closing bank balance as of the submission date
11	Transaction Date	Deposits	Withdrawals	Expense/ Deposit Category	Details ✔ Describe the purpose of the transaction (e.g., membership fees collected, event name, specific purchase). ✔ Include relevant names (e.g., vendor, event, or funding source). ✔ Be specific (avoid vague terms like "event costs" or "supplies").
