APPENDIX III: PERFORMANCE GOALS AND ASSESSMENT PLAN

For each academic performance or growth goal described in the Narrative, identify the assessments (both formative and summative) the proposed charter school will use to measure and monitor academic success. List the baseline and annual goals for all student subgroups, replicating rows 3 and 4 as many times as is needed to address all performance or growth goals. Appedix III should be submitted as Attachment 3 - Performance Goals and Assessment Plan.
