The following instructions describe how to complete the Intervention Plan, Goals, and Data tabs.
The data entered will automatically graph on the applicable graphs.

Intervention Plan
This tab contains the student's a description of the behavior and intervention details if applicable.

Step 1:Enter client's name in cell BC1.

Step 2:Enter qualitative descriptions of the target behaviors, intervention, and progress monitoring plan, cells BH4, BH6, BH8, BH10, BH12, and BH14.

GoalsThis tab is used to enter quantitative descriptions of the intervention and progress monitoring; goal(s) text is also entered on this tab.

Step 1:Enter intervention start and end dates in cells B2 and B3 following the data validation format MM/DD/YYYY.

Step 2:In cell B4, enter the total # of goals being tracked. The number needs to be between and including 1 to 10.

Step 3:Enter behavioral goal.

DataData for each goal is entered on this tab. Data should ONLY be entered in Baseline and Intervention columns. Always work horizontally across the chart (meaning complete Column A, then B, then C, etc.). It is recommended that you title Columns B, C, D, and E with the name of the actual behavior you are monitoring (example: talk-outs).

Step 1:In column A, enter/ locate the appropriate date for which data is to be entered.

Step 2:In column B, enter the baseline data for the first behavior you are monitoring. Baseline data should be collected PRIOR to starting an intervention.

Step 3:In column C, enter the data for the behavior you are monitoring AFTER an intervention has begun.

GraphsData entered into the data columns will automatically graph on the Behavior 1 Chart.

You can change the title, appearance, labels, etc within each graph as needed.
