Directions: Carefully read each indicator listed. Decide your level of agreement with

each proficiency statement. Enter a brief description of your understanding of each

indicator in the box you selected. Enter additional reflective comments related to

the standard at the end of each section.

repeat the process at the end of the program by taking the Post Assessment in the Post

Definition of Rating Levels:

0 = None No theoretical knowledge and understanding of the concept; no exposure

or job-related experience in applying the concept to my professional practice.

1= Very Limited: Minimal theoretical knowledge and understanding of the concept;

minimal exposure and little experience in applying the concept, which has not
22
been needed to any significant degree in my required job-related experiences.

2= Limited: Basic level lacking depth and breadth of theoretical knowledge

and understanding of the concept; which has not often been required in
25
my job-related experiences.

3= Very Good: Sufficient level of depth and breadth of theoretical knowledge and

understanding for effective application of the concept to professional

practice as frequently required in my job-related experiences.

4= Exceptional: Exemplary/Superior level of depth and breadth of theoretical knowledge
