Instructions: Submit this completed form with dates indicated, along with an emailed copy of your Supreme Columbian Award Form #SP-7 by March 15, 2026. The IL-SP7 and SP-7 submissions will complete your application for the Illinois in Action Award.

QUALIFYING REQUIREMENTS for the 2025-2026 fraternal year

To qualify for this award, a Council must:

1. Be in full compliance with Safe Environment requirements having the Grand Knight, Program Director, Community Director, and Family Director positions filled and training completed by March 15 of the current fraternal year.

2. Submit Form 185, 365, 1728 and both audits on Form 1295.

3. Bring in at least one new member this fraternal year by March 15, 2026. Transfers do not qualify.

4. Submit a completed copy of the Supreme Columbian Award Form SP-7 in addition to this application to be considered.

5. Contribute to at least 3 charitable funds for Illinois State Council Charities, Inc.

AWARD BRACKETS AND POINTS FOR AWARDS

Points are awarded for completing activities, recruiting efforts, charitable accomplishments, submitting paperwork, etc. Once all points are summed up, the Council may qualify for the Illinois in Action Award, based upon total accumulated points.

• 25 points for submitting all of the following forms: 185, 365, 1295 and 1728 to the State Office and to Supreme.

• 5 points for a Notice of Appointment of Round Table Coordinator (#2629) form filed.

• 10 points for an Annual Report KofC Roundtable (#2630) form filed.

• 25 points for filing Special Olympics form (#10784) with Supreme Council (ID Drive contribution to State counts!)

• 40 potential points for activities completed for the four (4) program sections of IL-SP7 (1 pt. for 1st, 2 for 2nd , 3 for 3rd , 4 for 4th for each of the four (4) sections for a total of 40 potential points).

• 5 points for each Form 10784 turned into State Council for Council programs for each Faith in Action program. Maximum 20 pts each for Faith, Family, Community and Life (80 pts total).

• 10 points for State Council Service Program of the Year (Form #STSP) entries – one per Faith in Action category, up to 40 points.

• 5 points for each Family of the Month (#10784) entry submitted to the State Office and Supreme (max 12 entries, up to 60 points).

• 10 points for a completed Family of the Year (#10680) entry submitted to the State Office.

• 5 points for each Knight of the Month (online form) entry submitted to State Office (max 12 entries, up to 60 points).

• 10 points for a Knight of the Year (online form) entry submitted to State Office.

• 10 points for each Exemplification of Charity, Unity, and Fraternity hosted or participated in for a maximum of 40 pts

• 10 points for each new member exemplified and reported during the current fraternal year and reported by March 15, 2025.

• 50 points for meeting McGivney Award criteria (Supreme Membership Goal).

• 50 points for meeting Founders Award criteria (Supreme Insurance Goal) goal for Fraternal Benefits Night participation.

• 10 points each for meeting State Charitable Goals in a charities category, with a 50-point bonus for reaching goals in all 5 charities. (100 pts total)

• 20 points for appointing a Charities Ambassador.

• 10 points each for participation in an Illinois State Council sanctioned program activity. Must be verified with submission of Form 10784.

• 50 points for starting or having a COR group that meets a minuimum of 4 times during the fraternal year.

Programs with an (*) are featured programs and are equal to completing two programs, if entirely completed, per Supreme's guidelines, so list these twice in the appropriate section of this form.

Note: Sixteen (16) points must be completed with at least four in each of the Faith in Action categories and reported to the Supreme Council via the form (#SP-7) to be eligible for the Columbian Award (and by extension, the Star Council Award).
