Adults Names get their own column. Use the abbreviated titles: Mr., Mrs., Miss, etc. unless the person is a Doctor. If the person is a Doctor, spell out the whole word "Doctor".

Children's Names (under the age of 18) get their own column. Girls have the title "Miss". Boys do not have a title.

Street Address gets its own column with ALL words completely spelled out, like "Street", "Avenue", "Boulevard", "Lane", "Northwest", "Southeast", etc.

Apartment/Unit Numbers get their own column with the full word "Apartment", "Unit", "Number", etc. spelled out.

City and State gets its own column with the full state name spelled out, like "Pennsylvania", "California", etc.

Zip Code gets its own column.

Spell everything EXACTLY as you would like it to appear on your envelopes.
