167 | Dr. Cristina Markiewicz | This past week was my first course on CBCT evaluations, I was overjoyed to see so many of my LVI instructors, friends and mentors taking the class alongside me. A quarter of the people taking the class had taken the course last year and took the class again because all of the additional things you can see on scans that help us to better diagnose and manage our patients. So many of the people I talked to who were taking it a second time were saying how they were able to see and understand so much more the second time around because it wasn't their first introduction to seeing the anatomy presented in the CBCTs and they were able to absorb more of the content. Bill took a lot of the time to point out clinical correlations and connections to our treatment, which is something I have never even seen offered by any other CBCT course. He also added a lot of pertinent information that had to do with ear issues like tinnitus and vertigo because of all the work he does talking to patients on the FB group seeking out solutions after failed treatment. I learned so much information from the CBCT course this past week. I was studying my binder on my flight home and just cracked it open to review some scans today. It's definitely taking me awhile to go through scans but with practice, hopefully I can go through them within 15 minutes like Bill can. Regardless of where you are at in your LVI journey, you need to take this class. If you practice with another dentist or associate, they also should take this class. It's invaluable. I hustled to keep up and Bill's passion is palpable...you all know this, but Bill's finest moments contributing to our profession for me have been the past few years...and I have known him for some time. Now...to address some of the elephant's in the room (there are a few of you)...YES...it can be overwhelming and I even know some have been frustrated with the amount of change the past few years. Changes in equipment and even protocols of HOW and WHY we do things in the clinic. Do not think for a second as a clinical instructor that we are any further ahead in some of these advancements than any of you...that is WHY you will see so many clinical instructors in the very courses you all are in...and in ALL cases taking these courses over and over. Even if Bill could master presenting EVERYTHING there is to know in ONE course...there is not a single person that could physically absorb ALL the information...NO ONE! Plus...as we have the best R/D at LVI with our clinics...there will always be Bill making these protocols better...that is a guarantee! So please quit gripping about some of these posts being another commercial for LVI...I think it just resonates for MANY that are posting that they are wanting to take even better care of our clients and helping you. If you are not finding this to be challenging lately than give up packing amalgams and join the physiologic bus. Seriously...some of these cases are tough and client's expectations are "huge"! Just read some of the comments on the social media forums when a dentist screws up...yes I have even gotten one of those...maybe more...sometimes my Team and I deserve it! Lol Finally...if you are practicing the mindset of being the best...you will have your share of failures. I know people get scared of making mistakes and "failing" because some of you dwell on worst cases scenarios...like getting "sued" or shit like that. Look...I get it...I practice in a small town and have been in this same location for 20 plus years. Trust me when I say...it is not an easy path...but stay tuned in because as you become more successful and "win" your challenge of staying in the zone of being your best...just got harder. I have been fascinated with CBCT scan for the past 8 years, have taken numerous courses in scan interpretation including spending personal time with Dick Greenan and spend so much time mapping and reading them which has been invaluable. Bill has taken everything to the next level and to the future! I was truly amazed at how much more analyzation there is to be seen and evaluated. This is not something you can look up online and figure out. Even if you do not spend time scouring scans like some of us do, this course may help you know when things are not going to respond to orthotic treatment as well as how to correct other issues to further help patients improve. Taking CBCT scans for any TMJ treatment, in my opinion has always been and should be the standard of care. I was so happy to have learned about the interpretation of the scans. I took the scans and although I did okay with them I didn’t understand all what I was looking at and why it was showing one thing vs. another. Bill showed us step by step what scans meant and there were so many things that became more clear to me. As an example he showed us from one Scan a way to see if you can recapture the disc or not. Now about the CBCT course....so I have had a CBCT machine for over 10 years and I was taking scans and basically using it to see if there was decompression of the joints and for implant planning. I had previously taken the CBCT course, which is flat out awesome, and a very critical course, as we include airway, TMD and Jaw Development Ortho for our patient's consideration. I took the Biopak mastery course in December, so I was a little guarded that the Scan Interpretation was a different course. It was excellent, and offered me way more than I expected, and once again, Bill you put together some of the very best manuals. I played golf with Bill after the course and we were talking about how time and effort was put into these courses. Not only by him, but by ALL the support team!! For the Advanced Scan Interpretation course, if you have the BioPak, you NEED to take this course. Bill goes into the very detail we need as physiologic dentists, to be able to read what we are scanning and proceed from there. If you don’t have the BioPak (and think you can get by with the K7 – YOU CAN’T!), what exactly are you waiting for? This machine is so much better than the old stuff and keeps improving. As for the CBCT course, Bill told me he studied anatomy for quite a while to be able to present this material. I was blown away by the course and what I didn’t know. I do not have a CBCT in my office. I know now what to request when I refer patients out to have an image taken. I also know what things to look for when the image comes back! It was an amazing review in head and neck anatomy! Here's the thing I realized: we, as physiologic dentists, being trained at LVI, are truly the best in the world. But, if you think you know all the info, or what you’ve taken is enough, you’re wrong. This past week verified it is imperative to take these courses, and audit them every 1-2 years, if you want to stay on top of the heap!! I could not believe all of the work he did getting this course together. The information was invaluable, and I don’t see how you can treat patients with TMD without this knowledge. Even if you don’t have a cone beam in your office, you need to know how to read and interpret these images.
