In general, the green boxes should be edited to the specifications of your build. Anything not green should generally be left alone, and the sheet will behave itself and calculate damage and toughness properly.
3
[1]Average damage per hit ([Max+Min]/2), not the "DPS" on the weapon.
4
[2]Skill % damage as displayed in the skill tree, but remember to adjust for in-combat skill points like Vex rune.
5
[3]Include in-combat crit chance gains, like Gar rune, as well as stuff that doesn't end up on the character sheet like Pact of Bone.
6
[4]If you copy a build tab and re-name it, you can put the name of another tab in this cell and compare damage and toughness vs the comparator tab data.
7
[5]Calculating DPS with sub-100% crit chance or vulnerable uptime is slightly complex because each hit has to be calculated as a potential crit and/or vuln. To ensure hits are calculated properly, include the word "Crit" or "Vuln" as appropriate under Source to make sure they find their way into the proper calculation.
8
[6]"All Damage Multiplier" and elemental Damage Multipliers are summed together, but they can be entered separately here for the sake of organization.
9
[7]Combinations that depend on each other should be incorporated together. This may require a little math. For example, Bloodless Scream does more damage while enemies are frozen. Similarly, Azurewrath accumulates damage while the enemy is frozen. Hence, unless Frozen uptime is 100% (it isn't), the multipliers should be combined so uptime and damage are precise. For this example, Mythic Bloodless Scream grants 325%[x] damage (4.25x), Azurewrath grants 120%[x] (2.2x), so combined, we'd enter: (4.25 x 2.2) x 100 - 100 = 835 into the cell as the combined multiplier. With uptime at 50%, the effective DPS multiplier becomes 5.175x. If we were to calculate them separately (50% uptime of 325%[x] and 50% uptime of 120%[x]), we'd only get a 4.2x multiplier.

[8]Any percent-based Armor or Resistance bonuses are summed into respective buckets, so the Armor and Resistance stats entered in this box need to be the raw sum of resistance stats before multiplers are included. This should be in the character sheet if you mouse over the displayed stat (which includes percentage bonuses).
11
[9]Be sure to include in-combat bonuses and enemy debuffs like Weakness.
12
[10]Again, include in-combat bonuses like Resolve.
