By filling this information sheet, I hereby agree and consent that CCAP may collect, use, disclose, and process my personal information and sensitive personal information provided by me or possessed by CCAP, for one or more of the purposes stated herein.

The personal information collected by CCAP may include, but is not limited to, the following (the "Information"):

a. Full Name
b. Company
c. Company Address
d. Position Title
e. Department
f. Contact Number
g. Email Address

I agree that all Information that I provide CCAP may be collected, profiled and processed internally. Also, I consent to the collection, transfer, processing and storage of information collected whether done within or outside of the Philippines.

Furthermore, the information I provide may be used by CCAP in any of the following manner:

1. Only for CCAP’s internal communications (email blasts, invitations, updates, follow ups, and call) for industry related purposes.

2. We also require your cell phone and work phone which will be used only if we need to contact you on any urgent conference-related matters.

3. Subject to applicable law, CCAP may share, transfer, and disclose the information to the following:
a. To the Government authorities, at their request and only when legally required to do so;
b. To protect the rights and interests of CCAP, business partners, customers or third parties, as may be required and permitted by law; and
c. To pursue legitimate interests, as may be required or permitted by law.

a. To be informed whether the information is being or has been processed. This includes processing through automated decision-making and profiling;
b. To request access to and correction, update, and change of the information;
c. To object to the processing of the information;
d. To suspend, withdraw, or order the blocking, removal or destruction of the information; and
e. To withdraw my consent at any time.
