Feel free to expand or contract cells in order to provide complete answers. Once each record has been completed, save it as an individual PDF file with the record name as part of the title. Then the individual files can be uploaded via the link provided below.

Please note that the Information & Certification page will need to be printed and scanned, as original signatures are required.

In addition to the forms included here, the following documents must also be submitted as part of your application:

Eagle Scout Certificate - If you have recently had a Board of Review, and you have not yet received your Eagle Scout certificate, a copy of your Advancement Report or a letter from your council office is also acceptable.

Personal Statement on what leadership means to you

Letters of Recommendation — Submit at least three letters, no more than two pages in length each, from:

A scout leader

A community member

A teacher/faculty member (if home schooled, a parent letter is allowed).

Optional - you may include one or two additional letters, no more than two pages in length from local community members who have special knowledge of the candidate's abilities
