"The purpose of Melo is to help healthcare staff to better understand and to work to reduce preventable harm from occurring due to the actions of patients who lack capacity to control their behaviour, and who are at risk of impulsive behaviour [behaviour that challenges] due to a medical condition. Melo is not intended to eliminate risks or harm from occurring.

This Hazard Log is concerned with the worst case scenario, that lost data regarding risk, could lead to a reduced ability of staff to understand and manage such behaviour.

Due to the controls in place, it is considered that the likelihood of problems with the Melo system contributing to a reduced ability to understand and manage such behaviour is low.
