For each step of the process, duplicate the tab and name it <step> - <interviewer>Steps: Screen / ITW1 / ITW2 / ITW3 etc. In the title of the file there'll be the candidate's name, and in the tabs, the interviewer one

Do NOT show your completed template to the next interviewer (strong bias). The only thing you need to tell him/her is Go/No Go and potential areas to check further.
