Total Cost (Column B) : the total cost of the item or service

GPP Fellowship Request (Column C): the amount of funds you are requesting for this item or service to be covered by the GPP fellowship

Personal Contribution (Column D): the amount of funds, if any, you can personally contribute (i.e. from a job, from family, etc.)

Funding Received by Additional Sources (Column E):  the amount of funds you already received or expect to receive from other grants or scholarships, if applicable.

Please note: Column C, D, and E should add up to the Column B, “Total Cost.”

Notes (Column F):  For each line item, explain how you came up with total cost and explain any other details on what those funds are covering.
