1. This form shall be used to establish the database of all active ARBOs with existing processing/ production covered under Village Level Processing Center Enhancement Project (VLPCEP) launched in CY 2009 and those that have transitioned to Village Level Farm-focused Enteprise Development (VLFED) Project from CY 2018 and its succeeding years of other functional/existing VLPCEP sites.

Basic information of the Organization

(1) Region - This refers to the region where the ARBO is located.
- Kindly use the drop down list provided in selecting the concerned region.

(2) Province - This refers to he province where the ARBO is located.
- Kindly use the drop down list provided in selecting the concerned province.

(3) District - This refers to the congressional district where the ARBO is located

(7) Name of ARBO - This refers to the complete/full name of the ARBO (please don't abbreviate - i.e San Eduardo Agrarian Reform Cooperative). The drop down list will be completed based on the names of ARBOs encoded by province.Uploading as a Google sheet will be by Region.
- Try to use a blank cell and write the name of the organization which is not included in the list.
- Then copy and paste (value) in the column of the Name of ARBO.

(8) ARBO ID Number - This refers to the identification number issued to the ARBO
- As reference, ID numbers should be taken from ITeMA/ITeASy results/provided by DARCO-Planning Service
