How to use this invoice template

Thank you for helping us with our research initiative! Please follow the instructions below to create an invoice for payment of your facilitator stipend.

IMPORTANT NOTE:Facilitators should complete and submit one invoice each for research programs.

1)Click on "File" in the document menu bar (below the document title at the top of the window), then select "Make a copy..." from the dropdown menu which appears.

2)Replace the file name with "Research Program Facilitator Invoice: " followed by your name.

3)Click on the "Facilitator Invoice Sheet" tab (the grey tab underneath this spreadsheet).

4)Overwrite text in the following cells with your contact details, as indicated:

E3:Enter your name

E4:Enter your email address

G4:Enter your phone number

5)Enter your payment information into the cells indicated:

C7:Enter the name of the person to whom payment should be made (e.g. your name, or the account holder's name if different).

C8:Enter the address to which your check should be mailed.

F7:Enter the date upon which you are completing the invoice.

6)Enter your program details as follows:

B12:Select your program location from the dropdown options in the cell.

D12:Enter the number of research programs facilitated.

F12:If you facilitated in a triad, change the total number of facilitators to 3. Otherwise, leave this cell as it is.

G12:Enter the price point that you set for your program registration fee.

E15:Enter the number of program participants who registered for and participated in your program, but did not participate in the research study (e.g. due to ineligibility), if applicable.

The total owed to you should now be displayed automatically in cell G7.

Additional income is split equally between you and your co-facilitator(s), and added onto your $600/program stipend.

7)Right-click on this tab (* How to use this template *) and click to delete the tab.

8)Click on "File" in the menu bar, then select "Download as ▶ PDF document (.PDF)"

9)Locate the PDF file on your computer and attach it to an email; use the subject line, "EBT Research Program Facilitator Invoice:" followed by your name.

10)Send the email to sam@eatbreathethrive.org.
