This worksheet can be used to identify health and safety hazards to comply with both ISO 45001 and IIPP. For each hazard, you must assign a quantitative score. This score can be used to determine if the risk is significant. Add additional controls to significant risks in order to reduce the score and show yoru auditor continual improvement. Don't forget to save each update as a new revision (or date controlled revision) so that you can demonstrate the evolution of your health and safety system.

This is the date in which the hazard was first identified. The identity of the person who has identified the risk. Tip: to meet the consultaiton and participation requirement of ISO 45001, have your workers identify at lease some of the health and safety hazardsThe process where the risk is presentDescription of the risk and its effect. i.e. Risk of back injury while unloading material.Operational controls can include administrative, PPE, training, etc...Give this a score of 1-10Give this a score of 1-10This is a product of impact x probabilityAssign a threshold. This is recommended for significant risks but additional operational controls can be implemented for any risk. The risk assessment should be reviewed at least annually, when there is a new or different process, in response to an incident, or if there is a new or different risk identified.
