Mark is a 22 year old male firefighter with Sickle Cell Disease and is being seen for a blood test for a possible blood transfusion. Through previous access to records and ability to see his history, a treatment path is chose for him. Following the blood transfusion, Mark follows up with the hematologist and is also referred for psychological and pain management. The complete spectrum of care, including social work, helps to maintain a normal lifestyle, not only physically, but emotionally and logistically.
A patient presents to the acute care setting and down through a referral broker system or bundle convener to send to a post-acute care provider. That referral would then be accepted by a SNF that would send the acceptance response back up the chain to the hospital to close the loop on the transition. The patient would be placed in the SNF while they continued their primary care visits with data accessed across the continuum. The patient leaves the SNF, but will continue case/care management to account for any time left in their bundle (120 day bundle). Through Care Coordination and interoperability at every point in the patient's journey the patient makes a full recovery and does not return to the hospital for the condition represented in their bundle.
