This form is to be completed before every Quidditch Canada event to ensure all risks are systematically identified, and either eliminated or controlled. This is then to be communicated as required. Ongoing reviews need to be conducted to ensure the risk has not increased. Should an accident, injury or near miss occur during an event, the Risk Assessment is to be revised and all changes communicated.

To complete the Risk Assessment:

1. List all the features for the event e.g. weather, fields, equipment etc

2. List all potential risks which could cause injury to anyone participating

3. To find the ranking of the risk areas, rate each risk’s ‘Probability of Occurring’ and ‘Consequence’ on the Risk Ranking scale. See the “Required Actions for Risk Rankings” table for each Risk Ranking.

4. Determine how you will minimize the risk and document the Control Measures. See the Quidditch Canada Risk Assessment Guidelines for examples.

5. Reassess the risk (unless it is risk ranked Low), to ensure that the control measures do not cause additional risks. Use the Risk Ranking scale, then determine if any further actions are required and implement appropriately.
