Late Fees Charged

10%Six (6) to twenty-nine (29) calendar days after expiration date, a late fee of 10% of the total renewal fee is due in addition to the renewal fee.Failure to submit a complete renewal application and appropriate fees to the Department by the license expiration date will result in late fees as indicated.

50%Thirty (30) to fifty-nine (59) calendar days after expiration date, a late fee of 50% of the total renewal fee is due in addition to the renewal fee.

75%Sixty (60) to eighty-nine (89) calendar days after expiration, a late fee of 75% of the total renewal fee is due in addition to the renewal fee.

Invalidated
If a license renewal application and appropriate fees are not received by day ninety (90) following the expiration of the license, the licensee shall cease operation and submit an initial application and associated fees to the Department in accordance with Chapter 2, Section 2.3.

Change of Ownership

The current licensee is required to notify the Department by submitting a Letter of Intent (LOI). An email notification will be sent with information on the CHOW application process. The CHOW Application and appropriate fees must be submitted by the prospective licensee at least 60 calendar days before the change of ownership. Requires prior approval by the Department. A new license will be issued.
