All candidates, including those with no expenses, must submit an updated version of this form to 4230--SI suite Talley by 5:00pm each class Monday between the All Candidates Meeting and the election as well as a final version by 8:15pm on the night of the election.

All receipts should be placed in an envelope and stapled to this form. All information must be accurate as of 5:00pm of the immediately precceding Friday, except for the final version, which shall reflect the entire campaign.

All candidates should be familiar with the campaign expense regulations contained in Chapter VI of the Board of Elections Handbook.
