1Issues encountered during testing to be documented on the "Testing Issues" worksheet.

2Each field on the "Testing Issues" tab contains "hover over" help text in the header field to explain the purpose of the field and available selections (where applicable) along with their descriptions.

3When a new issue is documented, information for the following fields is to be entered:

DateInclude the month, day and year that the issue was documented

SeverityThis denotes the impact to the system

PriorityIndicates how quickly the issue should be addressed

DescriptionProvide an explanation of what occured, the actions taken to create the issue and the expected result for the action. Also include a link to the document containing any screenshots that were taken to help convey the issue.

Reported ByThe name of the person who reported the issue

Assigned ToThe name of the person who is responsible for triage (initial assessment) and assignment of defects. Also, enter a Comment for the field which contains @ and the assigned person's name then select (check) the assign checkbox to send an email notification to the person assigned.

4After the initial assessment is made, the defect should be updated as indicated below:

Assigned ToThe value should be changed to the name of the person identified to resolve the issue or back to the Reported By person if the issue was determined to be invalid. Also, enter a Comment for the field which contains @ and the assigned person's name then select (check) the assign checkbox to send an email notification to the person assigned.

Status• In-Process: if the issue is being assigned for resolution
• Invalid: if the issue was determined to be related to a testing error
• Deferred: if the issue will not be fixed in the current, planned release of the system

5After the issue has been resolved and is available for re-test (if applicable), the following information should be entered:

Assigned ToThe value should be changed to the name of the Reported By person. Also, enter a Comment for the field which contains @ and the assigned person's name then select (check) the assign checkbox to send an email notification to the person assigned.

6Once the issue has been resolved, the Reported By person will verify and update the issue as indicated:

7The Reported By person will retest to verify that the issue has been resolved and update the issue as indicated below:

Status• Closed: If the testing was successful (i.e., issue has been verified as resolved and no longer occurs)
or
• Reopened: If the testing was NOT successful (i.e., issue has NOT been resolved and continues to occur)

Date ClosedEnter the month, day and year that the issue was closed

Assigned ToIf the issue status was changed to "Reopened", the value should be changed to the name of the person who implemented the resolution. Also, enter a Comment for the field which contains @ and the assigned person's name then select (check) the assign checkbox to send an email notification to the person assigned.

8The Comment field can be used to capture additional information (e.g., root cause, resolution applied, etc.) about the issue, as needed
