DAILY SYMPTOM/EXPOSURE MONITORING

Symptoms: Do you have any of the following symptoms?
* Fever of 100.0° Fahrenheit or higher or chills * New cough
* Shortness of breath * Fatigue from unknown cause
* Muscle or body aches from unknown cause * New onset of moderate to severe headache
* Sore throat * New loss of sense of taste or smell
* Vomiting or diarrhea * Congestion or runny nose

Close Contact/Potential Exposure: IDPH definition of close contact:
*Anyone who is within 6 feet of an infected person for a cumulative total of 15 minutes or more
over a 24 hour period, starting 2 days before the positive case’s symptoms or, for asymptomatic
positive cases, 2 days prior to the date of the test, until the time the positive case is isolated.

If a staff/student has any symptom of COVID-19, they should stay home and report their absence to the attendance line, which will also give the information found below about next steps.

A COVID-19 test is strongly recommended before a symptomatic person may return to school, regardless of vaccination status or past history of COVID infection. The test should be submitted to the school nurse. The symptomatic person must also be fever-free, vomiting-free, and diarrhea-free for 24 hours, and have a reduction in all symptoms. If the test is positive for COVID-19, the person will be excluded from school for 5 days, and may return on Day 6 provided they wear a mask on Days 6-10.

IDPH and McLean County Health Department allow a POC rapid antigen test at home or in an approved facility, or a PCR COVID-19 test at an approved facility to return to school. A picture or copy of the test should be provided to the school nurse or Principal before returning.

**HOME TESTS ARE ENCOURAGED AND ACCEPTED.

4
