Instructions: Read each line and mark Yes or No for each option. Answer the questions from who you are not, don't try to answer the questions based on the age group of each stage. For example: An infant stage maturity task is "I have experienced strong, loving, caring bonds with a mother/woman. Maybe when you were an infant you did not get that from your mother, but maybe when you got older, you had a teacher or other type of care giver who you were able to experience that with, so you would mark "Yes".
