Use this worksheet to help identify the risk levels of the hazards you identify. The Risk Level column will auto calculate based on your inputs into the Probability and Severity columns. Some generic Hazards have been listed, along with generic “risk reduction” ideas. The “How can I reduce my risk?” column is for brain storming ideas, followed by more specific actionable items which will be listed in the “Specific action to reduce risk?” column. This worksheet is intended to help organize ideas during the identification of hazards and should be referenced as you develop specific action plans for each hazard identified. After getting a risk number for each hazard the user should develop and implement preparedness planning in a “most risk” to “least risk” order.
